Clinical Nurse Specialist-Led Public Health Clinics: Building the Future of Integrated Population Health Services

ABSTRACT
Background: Healthcare systems worldwide face increasing demands arising from population ageing, chronic diseases, mental health disorders, maternal health challenges, workforce shortages, and rising healthcare expenditures. Traditional physician-centered models remain essential but are often insufficient to meet the growing need for preventive, continuous, and person-centered care. Clinical Nurse Specialists (CNSs), as advanced practice nurses with expertise in clinical care, leadership, education, research, and quality improvement, are uniquely positioned to address these challenges. However, their role remains underutilized in many healthcare systems.
Objective: To propose an integrated Clinical Nurse Specialist-led Public Health Clinic model designed to strengthen preventive healthcare, improve continuity of care, support multidisciplinary collaboration, and contribute to sustainable health systems.
Discussion: The proposed model integrates six core domains of CNS practice: advanced clinical care, health promotion and disease prevention, leadership, education, research, and quality improvement. Rather than functioning as isolated services, these domains operate within a coordinated public health framework capable of addressing maternal health, chronic disease management, mental health, lifestyle medicine, community engagement, and health equity. The model aligns with international priorities established by the World Health Organization regarding Universal Health Coverage, integrated peoplecentered care, and strengthening the health workforce.
Conclusion: Clinical Nurse Specialist-led Public Health Clinics represent an innovative strategy for improving healthcare accessibility, patient outcomes, workforce sustainability, and population health. Further implementation research and economic evaluations are recommended to determine effectiveness across different healthcare settings.
